This form enables you to sign up online for sponsorship
of the GICA 104th Annual Convention. We accept payment by check at the mailing address below, or you may call the GICA office with your credit card information for payment processing. DO NOT e-mail your credit card information — it is not secure to do so.

If you prefer to fill out and submit a hard copy of the sponsor form, you may download it, print it, fill it out and send it by mail or by fax to the GICA office. The GICA fax number is 281.992.4383.
Sponsorship forms must be received by August 1, 2009 to ensure recognition on printed materials prepared in advance of the convention. Please forward the following to tbestes@earthlink.net no later than August 1 for inclusion:
- your company logo in vector-based or high-resolution rasterized format, AND
- a photo, if desired, for inclusion in the sponsor slideshow, OR
- approval to use the same slide from the previous year (click here for reference) for your company's 2009 slide
